Information sent through USPS, fax, e-mail or telephonic communication does not establish a doctor-patient relationship.
AMC offers prospective patients a Free Consultation. The free consultation is just a +-15 min meeting with one of the doctors or providers of AMC to allow the prospective patient to tell us their story and describe their problem and for us to discuss our possible solutions and whether we think we can help the prospective patient. This free consultation does not establish a doctor-patient relationship. As such, No medical notes or patient encounter paperwork is made by the doctor(s) doing the free consultation.
From time to time AMC will run a special offer of a free examination. This free examination only happens after the prospective patient has completely filled out and signed the AMC entire intake paperwork, had a face-to-face new-patient history and new-patient examination from an AMC physician, the prospective patient enters into the AMC written patient agreement including our self-pay fee structure and other terms to represent the patient in a specific matter and finally the AMC physician and an AMC corporate officer having accepting the prospective patient as a patient by signing the patient agreement. Should all of the above in this paragraph take place then this prospective patient shall be accepted as a patient and establish a doctor-patient relationship.

All services are self-pay. That is because virtually all the services done by AMC and its Doctors are not covered by insurance anyway so we decided to go completely self-pay. Following the No Surprises Act of 2022 guidelines, AMC will provide you a complete written list of procedures and treatments and all associated cost for your approval before we start any treatment(s) so you will not have any unexpected medical expenses.
All appointments with AMC Doctors and Providers to include but not be limited to Helen Stoddart, MD; Greg Millar, DC; Joan Brown, DC; Kris Huggins, EP; or Bonnie Sims, ND MDiv, hereafter (the “Doctors and Providers”), are self-pay. Appointments with the AMC Doctors and Providers are not billed through insurance. AMC and the Doctors and Providers do not accept any insurance and do not file insurance paperwork on your behalf. AMC and its Doctors and Providers will provide you with a superbill receipt for services performed. This is not a guarantee of payment or reimbursement from anyone or any entity. AMC and the Doctors and Providers do NOT use traditional CPT codes for services rendered or traditions diagnosis or diagnosis codes. AMC and the Doctors and Providers are not in-network with, or out of network provider for or a provider for any insurance company or government insurance program including State of Alabama or Federal Government health insurance programs or Private Health Insurance programs including but not limited to BCBS, United Health Care, Aetna, Humana, Cigna, Veteran Administration (VA), Tricare, Medicare, Medicaid, Alabama Workers Comp or any others.
AMC and its Doctors and Providers do not accept or participate in the Medicare program. If you are a Medicare Part B beneficiary and wish to become or continue as a patient of AMC and its Doctors and Providers, you hereby accept the terms and conditions set forth herein as a Private Contract between YOU and the Doctors and Providers. This Private Contract provides that all appointments and services are self-pay, and absolutely no Medicare payment(s) will be made to you or to the Doctors and Providers or to AMC for the services provided, even if such services are covered by Medicare. You further agree not to submit any charges, payments, superbills or statements of treatment to Medicare for any reason as that would be fraud as all treatments fall under the Private Contract established herein. If you do not agree with this Private Contract then you will inform the doctor before any services are rendered and you will not become or continue as a patient of AMC.
The Medical Doctors at AMC DO NOT prescribe narcotic pain medications or Benzodiazepines. The medical doctors do prescribe other medications as needed for the treatment of our patients. By using other techniques and remedies to control your pain we strive to get you off all pain medications and benzos.
If your accepted as a patient by AMC and its Doctors then it is your responsibility to keep AMC and its Doctors and Providers up to date on any changes to your health. You further agree that before any treatments you will provide AMC and its Doctors and Providers information with full disclosure regarding any accidents, injuries or falls (no matter how small) since your last anointment with AMC. You will further provide AMC and its Doctors and Providers information with full disclosure on any and all doctor visits, hospital visits, diagnosis, any imaging or test ordered or completed or any change of medications since your last visit with AMC and its Doctors and Providers. Should you Not inform AMC and its Doctors and Providers of any of the above prior to any treatment and that subsequent treatment(s) by AMC injures you, then shall indemnify and hold AMC and its Doctors and Providers harmless.
